I understand that this application creates an agreement between me and the YWCA Westfield. I agree that the YWCA Westfield will not be responsible or liable in any way for damage to work or injury. I give my permission to the YWCA Westfield to take photos/videos of me, my work and/or both during the show and to use these for promotional purposes. I understand the YWCA Westfield reserves the right to make additional inquiries, request more documentation and refuse entry to individuals. I understand the decision of the YWCA is final.

Please see attached the layout of Moore Park where the show is held. The map shows buildings, walkways, and electric outlets. If you have attended the show in the past you may have a favorite location. If so, please indicate your preference on the map and return with your application and check. Preferences will be accommodated to the best of our ability based on a first come basis
